The Performance Paradox

The traits that make a man excellent at his work are the same traits that make him good at the early, quiet version of his own deterioration.

The men I worry about most are rarely the ones who look unwell. They are the ones who are visibly succeeding: running companies, building institutions, training hard, carrying more than anyone around them realizes. They have been rewarded their whole lives for a specific capacity, the ability to absorb load and keep functioning, and that capacity is exactly what lets the early disease go unaddressed.

This is the Performance Paradox. The skill set that produces external success is the same skill set that produces a quiet, well-managed decline. The better a man is at performing through distress, the longer his body can deteriorate without interrupting the performance.

Why the cath lab fills with high performers

Achievement and cardiovascular risk are not opposites. The driven, capable, high-output man often carries the precise profile that accelerates vascular aging: chronic time pressure, compressed sleep, deferred symptoms, a stress system that never fully stands down. He reads his own fatigue as discipline. He reads his 3 a.m. wakeups as a busy mind. He reads the chest tightness as something to revisit after the quarter closes.

The promotion that raised his blood pressure

The paradox compounds with success, not despite it.
