Is It Anxiety or My Heart? A Guide for Men Navigating Palpitations, Racing Pulse, and Chest Tightness

Palpitations, racing heart, and chest tightness can come from anxiety or from cardiac arrhythmia. Here is how men can tell the difference and what to do.

The heart beats, then it stumbles. Or races for thirty seconds without warning. Or you notice a tightness in your chest that feels like dread, but you cannot point to anything you are dreading. Your breathing goes shallow. Your hands tingle. You feel unreal, slightly removed from your body, like the floor has tilted. Then it passes. You feel fine. Embarrassed, even.

You tell yourself it was anxiety. You move on.

For some men, that conclusion is correct. For others, it is a missed opportunity to identify something in the heart’s electrical system that will keep returning, and in some cases, that warrants medical attention. The challenge is that anxiety and cardiac arrhythmia share so much physiological territory that distinguishing them on symptoms alone is genuinely difficult, even for physicians. This is not about catastrophizing. It is about being epistemically honest: some of what men call anxiety is not anxiety.

Why Men Get This Wrong More Often

Men dismiss cardiac symptoms at higher rates than women. Research from the National Institutes of Health and multiple emergency department studies has shown that men are less likely to seek care for cardiac symptoms, more likely to attribute them to stress, and more likely to wait for symptoms to resolve before mentioning them to a physician. The cultural pressure on men to not appear anxious or overly concerned about health is a real clinical variable with real clinical consequences.

The dismissal compounds in one specific direction: men who are anxious by disposition are less likely to take their cardiac symptoms seriously, because they have a ready explanation. “Of course my heart is racing, I am an anxious person.” But anxiety and arrhythmia are not mutually exclusive, and a diagnosis of anxiety does not immunize a man against developing a cardiac rhythm disorder.

There is also a directional problem in reasoning. The fact that your symptoms improve when you calm down does not prove they were caused by anxiety. Some arrhythmias are self-terminating. They end on their own. And their ending correlates with the moment your anxiety about them decreases, creating a spurious association.

The Physiology of the Overlap

Both anxiety and cardiac arrhythmia operate through the autonomic nervous system, which controls heart rate, blood pressure, breathing, and the distribution of blood flow to organs. Understanding this overlap is important because it explains why the two are so genuinely difficult to distinguish.

In anxiety, particularly panic disorder and generalized anxiety disorder, the sympathetic nervous system fires inappropriately. Adrenaline (epinephrine) and noradrenaline surge. The heart rate climbs. Blood is redirected away from the gut and toward muscles. Breathing becomes rapid and shallow, which can itself alter blood pH and produce tingling in the hands and feet. The heart may feel as though it is pounding, fluttering, or skipping.

In cardiac arrhythmia, the heart’s electrical system misfires. The result is a heart rate or rhythm that is abnormal independent of what the nervous system is doing. This abnormal rhythm then activates the sympathetic nervous system as a secondary event. You feel your heart misbehaving, which triggers anxiety, which feeds back into the sympathetic activation.

The result is that both conditions produce nearly identical constellations of symptoms: racing heart, palpitations, chest tightness, shortness of breath, dizziness, tingling, and a sense of impending doom or unreality. The phenomenology is so similar that even a detailed symptom history often cannot resolve the question.

The Arrhythmias Most Likely to Mimic Anxiety

Not all cardiac arrhythmias present the same way. Three in particular are common enough in middle-aged men and share enough symptom overlap with anxiety to warrant specific attention.

Supraventricular Tachycardia (SVT)

SVT is an umbrella term for arrhythmias that originate above the ventricles, usually in the atria or the AV node. The most common form, AVNRT (atrioventricular nodal reentrant tachycardia), produces sudden-onset, sudden-offset racing of the heart at rates that can exceed 180 beats per minute.

SVT classically begins abruptly, often described as a flip or a switch being thrown, and ends equally abruptly. The sudden onset distinguishes it from the more gradual acceleration of sinus tachycardia from anxiety. However, because SVT can trigger profound anxiety, and because episodes often self-terminate within minutes, many men attribute them to “panic attacks” and do not report them for months or years.

The distinction matters clinically. SVT is a structural electrical problem, not a psychiatric one, and it responds to specific interventions. A cardiologist can evaluate for SVT with ambulatory monitoring and, if captured, characterize the mechanism and discuss management options appropriate to your clinical picture.

Atrial Fibrillation (AF)

Atrial fibrillation is the most common sustained cardiac arrhythmia and its prevalence increases substantially in men after age 50. In AF, the atria beat chaotically at very high rates, and the ventricles respond irregularly. This irregularity, an “irregularly irregular” pulse, is the clinical hallmark.

Men with paroxysmal AF (episodes that start and stop spontaneously) often describe episodes of palpitations, racing heart, breathlessness, and a feeling of being unwell that resolves spontaneously. The resemblance to anxiety is close enough that paroxysmal AF is sometimes dismissed for months before a diagnosis is made.

The relationship between AF and anxiety is bidirectional. Anxiety symptoms are prevalent in men with AF, and untreated anxiety may worsen AF burden. Treating one without acknowledging the other leaves both poorly managed.

Premature Ventricular Contractions (PVCs)

PVCs are early, out-of-sequence contractions originating in the ventricles. They are extremely common and in isolation are often benign. But the sensation they produce, a pause followed by a forceful beat, often described as a “thud” or a “flip-flop,” is viscerally alarming and frequently interpreted as the heart “stopping” or “skipping.”

Men who experience frequent PVCs often present with anxiety about their heart, which leads to a chicken-and-egg diagnostic tangle. Anxiety increases sympathetic tone, which can increase PVC frequency. More PVCs increase anxiety. The loop is self-sustaining.

The key question is whether PVCs are occurring in isolation or in runs, and whether they are suppressed or worsened by exercise. PVCs that increase with exercise or occur in salvos warrant more careful evaluation than isolated PVCs at rest.

What Normal Cardiac Workup Looks Like

If you bring episodic palpitations, racing heart, or chest tightness to your physician, the initial evaluation will generally include the following.

Resting ECG: A 12-lead ECG captures your heart’s electrical activity at rest. It can identify certain arrhythmias, conduction abnormalities, and structural patterns that provide diagnostic clues. However, it covers only a few seconds of cardiac activity. A normal ECG during an asymptomatic interval does not rule out episodic arrhythmia.

Basic laboratory work: Your physician may check thyroid function (hyperthyroidism is a common and treatable cause of palpitations and racing heart), electrolytes including potassium and magnesium (imbalances trigger arrhythmia), a complete blood count (anemia causes tachycardia), and sometimes a cortisol level if adrenal pathology is suspected.

Echocardiogram: An echo provides information about cardiac structure and function. Certain arrhythmias are more likely in the context of structural heart disease (reduced ejection fraction, valve abnormalities, enlarged chambers). If your arrhythmia is recurrent or your symptoms are significant, a cardiologist may request one.

Ambulatory cardiac monitoring: This is where the diagnosis is most often made.

The Role of Ambulatory Monitoring: Holter and Event Recorders

The central diagnostic challenge is this: most arrhythmias are episodic. They are not present when you are sitting in a physician’s office having an ECG. The solution is to record cardiac electrical activity for extended periods in your normal environment.

Holter monitor: A Holter monitor records every heartbeat continuously over 24 to 48 hours. It is useful when symptoms are frequent, daily or near-daily, and likely to be captured within the monitoring window. It produces a large dataset that a cardiologist reviews for rhythm abnormalities, rate trends, and arrhythmia burden.

Event recorder / cardiac event monitor: For men whose symptoms occur less frequently, a 30-day event recorder is more useful. These devices record continuously in a loop but only save data when triggered, either by the patient pressing a button during symptoms or automatically when the device detects an abnormal rate or rhythm. Wireless event recorders transmit data directly to a monitoring center for review.

Implantable loop recorder (ILR): For men with rare but significant symptoms, an ILR is a small subcutaneous device implanted under local anesthesia that records cardiac rhythm continuously for up to three years. It is typically reserved for cases where symptoms are infrequent but clinically important, such as unexplained syncope or highly suspected but uncaptured arrhythmia.

The diagnostic yield of ambulatory monitoring increases substantially with monitoring duration. A 2011 New England Journal of Medicine study on cryptogenic stroke found that 30-day monitoring detected AF approximately five times more often than 24-hour Holter recording. For men with episodic palpitations, the duration of monitoring should be matched to the frequency of symptoms.

The Panic-During-Exertion Pattern: A Red Flag

This article focuses on episodic symptoms at rest or with minimal exertion, which covers the majority of anxiety-arrhythmia overlap. However, one pattern should be flagged separately because it carries a different risk profile.

If your symptoms occur specifically during exercise, rather than before or after, this is a distinct clinical scenario. Arrhythmias provoked or worsened by physical exertion can reflect ventricular origin, ischemic trigger, or structural heart disease that is not present in arrhythmias triggered by rest or stress.

Holding Both Diagnoses Simultaneously

One of the most clinically important concepts in this space is that anxiety and cardiac arrhythmia can coexist, and in fact commonly do. A man can have both generalized anxiety disorder and SVT. He can have both panic disorder and paroxysmal AF. The presence of an anxiety disorder does not make arrhythmia less likely; some data suggests it may modestly increase the risk.

This means that a normal cardiac workup is not the end of the conversation. If a thorough evaluation finds no arrhythmia, the appropriate response is not to conclude “it was just anxiety all along” and stop there. It is to take the anxiety diagnosis seriously, address it through evidence-based means, and remain alert to symptom changes that might warrant renewed cardiac investigation.

Equally, a man who is diagnosed with an arrhythmia should not assume that managing the arrhythmia will fully resolve his symptoms. If a significant anxiety component exists, it may require its own attention, both for quality of life and because anxiety itself is an independent cardiac risk factor.

Research published in the European Heart Journal has found that anxiety and depression are associated with a roughly 30% increase in incident atrial fibrillation. The mechanism is not fully established but likely involves autonomic dysregulation, inflammatory pathways, and behavioral factors including sleep disruption and alcohol use. Treating anxiety in men with cardiac symptoms is not merely about comfort. It may be part of the cardiac management strategy.

When to Seek Evaluation Without Waiting

Certain features of palpitations or related symptoms should prompt you to contact your physician sooner rather than later.

Symptoms that occur with exertion should not wait. Palpitations associated with near-fainting or actual fainting (syncope) are urgent. Chest discomfort that radiates to the arm, jaw, or back alongside palpitations requires prompt assessment. A heart rate that remains above 150 beats per minute for more than several minutes without an obvious cause such as intense exercise is worth evaluating urgently.

The pattern that should not be normalized: frequent episodic symptoms occurring more than once per week that you have been attributing to stress without ever having a cardiac evaluation. Not because the anxiety explanation is wrong, but because it has not been tested.

Having a Productive Clinical Conversation

Men who describe palpitations to their physician often receive one of two responses: reassurance (“it sounds like anxiety”) or immediate cardiac workup. The quality of the clinical conversation affects which direction it goes.

To be heard accurately, it helps to be specific. What exactly does the sensation feel like, a racing, a fluttering, a skipping, or a pounding? When exactly does it occur, at rest, in bed, after caffeine, during conflict? How long does each episode last, seconds, minutes, longer? Does it start and stop suddenly, or does it build and fade gradually? Have you ever felt lightheaded or nearly fainted during an episode? Have you ever tried to take your pulse during an episode?

These details are not just clinical data. They communicate to your physician that you are an accurate observer of your own body, which changes how your symptoms are weighted.

If you have already been told it is anxiety and it continues, you are entitled to say: “I have tried to manage the anxiety and the symptom persists. I would like to understand whether we have adequately ruled out a cardiac arrhythmia, specifically with ambulatory monitoring.”

Q: How can I tell at the time of symptoms whether this is my heart or anxiety? A: You generally cannot, and that is the honest answer. A few clues are suggestive: SVT tends to start and stop abruptly, while anxiety typically builds and fades more gradually. An irregular pulse during symptoms points more toward AF than anxiety. But these features are not reliable enough to replace clinical evaluation. The more useful move is to try to count your pulse during an episode and note the characteristics.

Q: If my ECG was normal, does that mean my heart is fine? A: A resting ECG is normal in the majority of people with episodic arrhythmias, because most arrhythmias are not present during the seconds captured by an ECG. A normal ECG is reassuring but does not rule out SVT, paroxysmal AF, frequent PVCs, or other episodic rhythm disorders. Ambulatory monitoring over days or weeks is a more appropriate test for episodic symptoms.

Q: Can anxiety actually cause palpitations that feel exactly like arrhythmia? A: Yes. Anxiety drives sinus tachycardia (the heart’s natural pacemaker firing faster), which can feel very similar to a pathological arrhythmia. Anxiety also increases PVC frequency in some individuals. The difference is that anxiety-driven palpitations are benign from an arrhythmia standpoint, even if they are distressing. But again, the only way to know which is driving the symptoms is adequate cardiac monitoring.

Q: I have been diagnosed with panic disorder. Should I still get a cardiac workup? A: Yes, if you have not had one that included ambulatory monitoring. A psychiatric diagnosis does not confer cardiac immunity, and panic disorder and cardiac arrhythmia coexist in a meaningful proportion of patients. A cardiologist who knows you have panic disorder can still provide an appropriate evaluation and help you determine whether your symptoms have been adequately characterized.

Q: Does caffeine or alcohol make arrhythmia worse? A: Both have well-documented associations with arrhythmia provocation in some individuals. Caffeine at high doses increases adrenergic tone and can trigger PVCs and SVT in susceptible individuals. Alcohol, particularly in excess, is one of the most common triggers for paroxysmal atrial fibrillation, a phenomenon well enough established to have its own colloquial name in the literature. If your symptoms cluster after caffeine or alcohol, this is relevant information to share with your physician.
