What a Cardiologist Checks in Women at 40: The Cardiovascular Baseline That Actually Matters
A cardiologist explains what cardiovascular baseline testing women should have at 40, what sex-specific risk factors to disclose, and what screens miss.
Why 40 Is the Meaningful Inflection Point
For most women, the cardiovascular risk conversation begins too late. The standard physicals of the 30s rarely include anything beyond total cholesterol and blood pressure. By 40, several things have changed simultaneously: risk accelerators from reproductive history are now quantifiable, the perimenopause transition has begun for a meaningful fraction of women, and silent atherosclerosis has had 15 to 20 years to develop in those with genetic or metabolic vulnerabilities. The Pooled Cohort Equation, the standard risk calculator used in most offices, begins generating numbers at 40 that can actually inform treatment decisions.
The problem is that the standard cardiovascular assessment at 40 is designed around a population that is predominantly male and predominantly older. It measures what is easy to measure, not what is most informative for a 40-year-old woman with a complex reproductive and metabolic history. The result is a systematic underestimation of risk in a population where early intervention produces the greatest long-term benefit.
What the Standard Screen Misses
The typical cardiovascular check at an annual physical includes a basic metabolic panel, a standard lipid panel with total cholesterol, LDL, HDL, and triglycerides, and a blood pressure measurement. This captures frank dyslipidemia and obvious hypertension. It misses several things that matter substantially for women.
LDL cholesterol is an imprecise marker. It is a calculated value, not a measured one in most laboratories, and it underestimates risk in women with small, dense LDL particles, which are more prevalent in women with insulin resistance or metabolic syndrome. Apolipoprotein B (ApoB) directly measures the number of atherogenic lipoprotein particles, including LDL, VLDL, and IDL, and is a stronger predictor of cardiovascular events than LDL in most studies. Lipoprotein(a), or Lp(a), is a genetically determined lipoprotein elevated in approximately 20 percent of the population. It is not correlated with LDL, is not reduced by statins, and is not measured in a standard lipid panel unless specifically ordered. Elevated Lp(a) above 50 mg/dL or 125 nmol/L is an independent cardiovascular risk factor that changes risk category regardless of other values. A single lifetime measurement is sufficient, as Lp(a) levels are genetically fixed and change little with diet, lifestyle, or most medications. High-sensitivity CRP (hs-CRP) is an inflammatory marker that adds cardiovascular risk information beyond the lipid panel, particularly in women. The JUPITER trial enrolled women with LDL below 130 mg/dL but elevated hs-CRP above 2 mg/L and demonstrated that rosuvastatin reduced major cardiovascular events by 44 percent compared to placebo. (Ridker et al., NEJM 2008) 5 / Solid For women with autoimmune conditions, PCOS, or metabolic syndrome, hs-CRP is often persistently elevated and provides a reason to initiate treatment in a patient whose calculated risk score would otherwise fall below the treatment threshold.
Fasting insulin and HOMA-IR measure insulin resistance directly. HbA1c does not detect insulin resistance reliably in the perimenopausal window, where hormonal fluctuations can produce normal HbA1c values in women who already have significant insulin resistance and are accumulating cardiovascular risk. HOMA-IR above 2.0 in a fasting state indicates insulin resistance; above 2.5 warrants clinical attention.
The Reproductive History Review
The cardiovascular risk assessment should include a complete reproductive history, and most do not. This is a structural gap. Obstetricians document pregnancy complications but do not always communicate their cardiovascular implications to primary care. Primary care physicians often do not know to ask. Cardiologists rarely see women young enough to use the information prospectively.
Preeclampsia is the most consequential item in the history. Women who had preeclampsia have twice the lifetime risk of stroke and cardiovascular disease and four times the lifetime risk of hypertension compared to women with normotensive pregnancies. (Bellamy et al., BMJ 2007) 5 / Solid The ACOG and AHA both recommend lifelong cardiovascular surveillance starting at the postpartum visit and continuing annually. In practice, this rarely happens because no one owns the follow-up. The woman is discharged from obstetric care and her primary care physician may not know she had preeclampsia.
Gestational diabetes confers a sevenfold increased risk of developing type 2 diabetes within 10 years of the index pregnancy and a doubling of future ASCVD risk. (Kim et al., Diabetes Care 2002) 5 / Solid The ADA recommends testing for diabetes 6 to 12 weeks postpartum and every 1 to 3 years thereafter. Many women receive the 6-week test but fall out of follow-up before the elevated risk materializes.
Recurrent pregnancy loss, defined as two or more consecutive losses before 20 weeks, is associated with antiphospholipid syndrome in approximately 15 to 20 percent of cases. Antiphospholipid syndrome is a prothrombotic autoimmune condition that substantially increases arterial and venous thrombosis risk and is associated with premature stroke and MI in women. This diagnosis requires specific antibody testing that is not part of any standard cardiovascular screen.
Polycystic ovary syndrome (PCOS) affects approximately 10 percent of women of reproductive age. It is associated with insulin resistance in 50 to 75 percent of cases, dyslipidemia characterized by low HDL and elevated triglycerides, and a three-fold increased risk of developing type 2 diabetes. Women with PCOS have a metabolic risk profile that substantially exceeds their standard calculated cardiovascular risk score. Androgen excess in PCOS also promotes adverse body fat distribution toward the visceral compartment. (Dokras et al., Circulation 2016) 4 / Promising
Early Menopause
Women who undergo natural menopause before age 45, or surgical menopause at any age, carry substantially higher cardiovascular risk than women with menopause at the median age of 51. The Nurses’ Health Study demonstrated that surgical menopause before 45 was associated with a 2.2-fold increased risk of coronary heart disease. (Wellons et al., Menopause 2012) 4 / Promising
The clinical implication is practical: for risk scoring purposes, adding 10 years to the chronological age of a woman with early menopause provides a more accurate risk estimate than using actual age. A 45-year-old woman who underwent menopause at 40 should be assessed as if she were 55 from a cardiovascular risk standpoint. This reclassification frequently changes the statin initiation decision.
Autoimmune Conditions
Women carry the majority of autoimmune disease burden. Lupus affects 9 women for every 1 man. Rheumatoid arthritis affects women at 3 times the rate of men. Psoriasis, while more equal in prevalence, produces inflammatory burden that accelerates atherosclerosis.
In systemic lupus erythematosus (SLE), young women have a 5 to 10-fold higher risk of MI compared to age-matched women without SLE. The mechanism is not entirely mediated by traditional risk factors; systemic inflammation, immune complex deposition, accelerated atherosclerosis, and treatment effects from glucocorticoids all contribute. Rheumatoid arthritis increases cardiovascular risk by 48 to 70 percent compared to the general population and is now considered a risk equivalent to type 2 diabetes by the 2019 ACC/AHA guidelines. Women with RA should be assessed as high-risk regardless of their standard Pooled Cohort Equation score.
Blood Pressure Considerations at 40
A single office blood pressure reading at an annual physical is often uninformative. White coat effect, defined as office BP more than 20 mmHg higher than home readings, is present in approximately 30 percent of patients and is more common in women and older adults. Treating white coat hypertension with medication exposes patients to side effects without cardiovascular benefit.
Masked hypertension, where office BP is normal but home or ambulatory BP is elevated, is present in approximately 15 percent of patients and is associated with target organ damage and cardiovascular events equivalent to sustained hypertension. It is often missed because only office readings are recorded. Home BP monitoring, with a validated device and a standard protocol of two readings twice daily for 7 days, provides a far more accurate picture than any office measurement.
Blood pressure tends to rise during the perimenopause transition independently of weight gain. Women who had normal BP at 38 often have mildly elevated readings at 42 not because of lifestyle changes but because of estrogen withdrawal’s effect on vascular tone and sodium retention. Tracking the trajectory matters as much as any single value.
Glucose and Insulin Resistance
HbA1c becomes less reliable as a marker for early metabolic dysfunction in the perimenopausal window. Hormonal fluctuations can produce HbA1c values in the normal range (below 5.7 percent) while significant insulin resistance is already present. Fasting insulin combined with fasting glucose, expressed as HOMA-IR, is more sensitive in this population.
HOMA-IR above 2.0 indicates early insulin resistance. This is clinically significant not because it predicts diabetes in the near term but because insulin resistance is associated with dyslipidemia, endothelial dysfunction, and ASCVD risk elevation that precedes any glycemic abnormality by years to decades. A woman with HOMA-IR of 3.0 and a standard HbA1c of 5.5 percent has clinically significant metabolic risk that is invisible to the standard screen.
An oral glucose tolerance test (OGTT) is more sensitive than HbA1c for detecting impaired glucose tolerance in women with a history of gestational diabetes or PCOS. The 2-hour post-load glucose value catches impaired glucose tolerance in approximately 30 percent of women who have normal fasting glucose values.
Coronary Artery Calcium Scoring
The coronary artery calcium (CAC) score is a low-dose, unenhanced CT scan of the chest that quantifies calcified plaque in the coronary arteries. It is the single most powerful reclassifier of intermediate cardiovascular risk. A CAC of zero in a patient with a calculated 10-year risk of 5 to 7.5 percent effectively reclassifies her risk downward and can be used to defer statin initiation. A CAC above 100 or above the 75th percentile for age and sex reclassifies upward and supports statin initiation even in patients who fall below the standard treatment threshold.
CAC is not universal screening at 40. It is specifically warranted when risk is uncertain and the result would change a treatment decision. In women at 40, the indications include: a history of preeclampsia, early menopause before 45, PCOS with metabolic features, a family history of premature coronary artery disease before 65 in a mother or before 55 in a father, or a calculated 10-year risk of 5 to 10 percent where the decision to treat is genuinely uncertain. The radiation dose from a CAC scan is approximately 1 mSv, equivalent to a few months of background radiation.
The multi-ethnic study of atherosclerosis (MESA) demonstrated that CAC improved risk prediction substantially over clinical risk factors alone and reclassified a significant proportion of intermediate-risk patients in both directions. (Detrano et al., NEJM 2008) 5 / Solid
What to Tell Your Cardiologist
Seven items every woman should bring to a first cardiovascular assessment:
A complete pregnancy history, including any complications. Not just whether she had children, but whether pregnancies were uncomplicated, whether there was preeclampsia, gestational hypertension, gestational diabetes, or fetal growth restriction.
Family history of cardiovascular disease in first-degree relatives, including the age at which events occurred. Premature disease, defined as before 65 in a female relative or before 55 in a male relative, substantially changes risk assessment.
Prior blood pressure readings. If she has home readings from the past 6 months, these are more informative than any office measurement.
Prior lipid panel results, ideally with the full panel rather than just total cholesterol. Trend over time matters; a cholesterol level that was normal at 35 but has risen to borderline at 40 warrants different attention than one that has been borderline for a decade.
Current medications, including oral contraceptives and hormone therapy. Combined oral contraceptives raise blood pressure slightly in most women and substantially in some, and also increase the risk of venous thromboembolism in women with underlying thrombophilias.
Any autoimmune diagnoses, even if they seem unrelated to the heart. Lupus, RA, psoriasis, and inflammatory bowel disease all carry cardiovascular implications that need to be incorporated into risk assessment.
Current sleep quality. Sleep-disordered breathing is underdiagnosed in perimenopausal women, where it presents atypically with insomnia and night sweats rather than the snoring and witnessed apneas more typical of male presentation. Untreated sleep apnea is an independent cardiovascular risk factor.
Who Stays with Primary Care, Who Sees Cardiology
The majority of women at 40 with no specific risk factors or history can have their cardiovascular baseline established with a thorough primary care visit that includes the expanded labs described above. The standard of care does not require a cardiology referral as a universal step.
The following women warrant a cardiology referral before 50: any history of preeclampsia with severe features or early onset before 34 weeks; PCOS with multiple metabolic features; known autoimmune disease; family history of premature coronary artery disease; elevated Lp(a) above 50 mg/dL; borderline risk score that does not clearly resolve even after adding reproductive and inflammatory risk enhancers; and any woman who has had a cardiac symptom, including chest discomfort, unexplained dyspnea on exertion, or palpitations, that has not been fully evaluated.
What the Assessment Should Produce
A cardiovascular baseline at 40 is not a one-time event. The assessment should generate a documented 10-year risk estimate that incorporates sex-specific enhancers beyond the standard calculator, a clear decision on statin initiation or deferral with a specified review date, a home blood pressure baseline, a target for repeat lab testing (ApoB and hs-CRP annually, fasting insulin every 2 to 3 years, Lp(a) once), and a record of the reproductive history that can inform future clinical decisions.
The goal is not to generate anxiety about a condition that has not yet developed. The goal is to establish a baseline while the atherosclerotic process is still early, modifiable, and often reversible with intervention. The women who benefit most from this conversation are not those who already know they are high risk. They are the ones who believe they are low risk because no one has told them otherwise.
The Resting Electrocardiogram
A resting 12-lead ECG at the cardiovascular baseline visit is not a screening test for coronary artery disease. It does not detect atherosclerosis and should not be ordered with that expectation. It is a reference document.
What it captures is structural and electrical baseline: QTc prolongation, which matters because women have a longer QTc at baseline than men and are at higher risk for drug-induced QT prolongation with antibiotics, antifungals, antihistamines, and antidepressants; LVH pattern suggesting longstanding hypertension that was not captured clinically; WPW or other pre-excitation patterns that explain palpitations; and left axis deviation that may indicate prior fascicular block.
Having a baseline ECG before starting any medication known to prolong the QT interval is clinically useful. Many women at 40 are on or will be placed on medications in this category, and a baseline QTc below 440 ms in a woman with a low-risk clinical profile means subsequent prolongation is attributable to the drug rather than assumed to be chronic.
Following Up: What the Baseline Visit Starts
A cardiovascular baseline assessment produces a record, not a conclusion. The most valuable output is a clearly documented plan for follow-up timing: when to repeat ApoB and hs-CRP, when to repeat fasting insulin if the baseline HOMA-IR was borderline, how frequently to check home blood pressure, and at what point a CAC score would change the statin decision.
Women who leave the baseline visit with a written plan of what to monitor and when are far more likely to receive timely intervention if their risk trajectory changes than women who receive a verbal “come back if you have symptoms.” The heart disease that kills women in their 50s and 60s is almost always building in their 40s, and almost always detectable with the right tests ordered at the right time.
